For each question, rate your organisation’s current practices on a scale from 1 to 5, where:
1 = Ground Zero – there is lack of clarity OR no systems or practices in place OR significant issues exist
2 = Emerging – we've taken Initial steps but processes or communication is inconsistent or underdeveloped
3 = Developing – our systems are functional but need review, refinement or improvement
4 = Established – our practices are well-implemented and generally effective
5 = Summit Peak – our practices are consistently applied, staff are consulted resulting in continuous improvement.
